The I-693 Immigration Medical Exam

What is Form I-693?
Form I-693, Report of Immigration Medical Examination and Vaccination Record, is the medical exam required by USCIS for most applicants adjusting status to lawful permanent resident (green card). It must be completed by a USCIS-designated Civil Surgeon. The exam confirms you do not have a health condition that would make you inadmissible to the United States.
Why it must be a Civil Surgeon
Only physicians who have been individually designated by USCIS may complete Form I-693 for applicants inside the United States. (Outside the U.S., Panel Physicians designated by the Department of State perform a similar exam.) Dr. Manghnani holds the USCIS Civil Surgeon designation.
How long the appointment takes
Most appointments take 30 to 60 minutes. If we need to administer vaccinations or you need additional tests, plan for slightly longer.
What's included in the exam
- Review of your vaccination history and prior medical records
- Complete physical examination (vitals, vision, general physical)
- Blood tests for syphilis (RPR/VDRL) and gonorrhea (NAAT) as required by USCIS
- Tuberculosis screening — IGRA blood test (or TST skin test where appropriate)
- Mental health and substance use screening required by USCIS
- Review of required vaccinations and administration of any that are missing
- Form I-693 completed, signed, and placed in a sealed envelope
After your exam
Dr. Manghnani will hand you the sealed envelope containing your completed Form I-693. Do not open the envelope — USCIS requires it to remain sealed. Submit it together with your adjustment of status application or to the USCIS field office as instructed in your notice. Form I-693 is valid for 2 years from the date Dr. Manghnani signs it, so it remains valid throughout most adjudication timelines.
If you need a follow-up visit
If a test result requires a follow-up, or if your vaccination schedule requires another dose at a later date, we will let you know at your appointment. Most patients complete everything in a single visit.
