A flat fee for the exam, with labs and vaccines billed separately based on your needs.

What's included in the $350 fee
- Complete physical exam by the Civil Surgeon
- Review of vaccination and medical history
- Form I-693 completed and signed
- Sealed envelope per USCIS requirements
- Required mental health & substance screening
Family discount: For families of more than 2 people having their I-693 exams together, we offer a $50 discount for each additional family member.
What's billed separately
- Blood tests for syphilis and gonorrhea
- Tuberculosis screening (IGRA blood test or skin test)
- Chest X-ray (only if required based on TB screening)
- Required vaccinations not already documented
We will tell you what each item costs before performing it so there are no surprises. Many patients arrive with up-to-date vaccination records and avoid most of these charges.
Payment methods
We accept Cash, Card (credit / debit), Zelle, and HSA (Health Savings Account — most plans cover this exam). Payment is collected at the time of service. The I-693 exam is generally not covered by health insurance because it is not medical treatment, but HSA funds typically can be used.
