Answers to the most common questions about the USCIS I-693 medical exam at I693 Clinic.
A Civil Surgeon is a physician individually designated by USCIS to perform the Form I-693 medical examination for applicants who are adjusting status inside the United States. A Panel Physician is designated by the U.S. Department of State and performs a similar exam for applicants applying for an immigrant visa outside the U.S. If you are inside the U.S. and applying for a green card via adjustment of status, you need a Civil Surgeon. Dr. Manghnani is a USCIS-designated Civil Surgeon.
Form I-693, Report of Immigration Medical Examination and Vaccination Record, is the medical exam required by USCIS for most applicants applying for adjustment of status to lawful permanent resident. It documents that you have been examined and meet the health-related requirements for admissibility to the United States.
Our exam fee is $350 per person, flat. This covers the examination itself, the paperwork, and the sealed envelope. Lab tests, TB testing, and vaccinations are billed separately based on what you need. Families of more than 2 receive $50 off each additional family member.
Most I-693 appointments take 30 to 60 minutes. If we need to administer vaccinations or run additional tests, the visit may take slightly longer.

Yes. USCIS requires Form I-693 to be submitted in a sealed envelope. Dr. Manghnani will complete and sign your form, place it in the sealed envelope, and hand it to you. Do not open the envelope — USCIS requires it remain sealed until they open it.
Form I-693 is valid for 2 years from the date the Civil Surgeon signs it. As long as USCIS receives the sealed form within that 2-year window, it remains valid throughout the adjudication of your case.
Bring a government-issued photo ID, your passport, Form I-693 (downloaded from the USCIS website and partially filled out), any vaccination records you have, a list of current medications, and payment for the exam. If you wear glasses or contact lenses, bring them as well.
USCIS requires age-appropriate vaccinations including measles, mumps, rubella (MMR), tetanus and diphtheria toxoids (Td/Tdap), pertussis, polio, influenza (seasonal), hepatitis A and B, varicella, meningococcal, pneumococcal, rotavirus, Haemophilus influenzae type B (Hib), and others as applicable to your age. The exact list changes from time to time. We will review your records and tell you which vaccinations you still need.
This is very common, especially for applicants who grew up abroad. You have two options: we can draw blood titers to check whether you already have immunity to certain diseases (avoiding repeat vaccinations), or we can simply administer the required vaccines on the spot. We'll explain the cost of each option and help you decide what's right for you.
USCIS requires blood tests for syphilis (RPR or VDRL) and gonorrhea (NAAT), and tuberculosis screening (IGRA blood test in most cases, or a TST skin test). The exam also includes USCIS-required mental health and substance use screening.
USCIS now generally requires the IGRA blood test (such as QuantiFERON or T-Spot) for applicants 2 years of age and older. The traditional skin test (TST) is acceptable for younger children. If your IGRA is positive, a chest X-ray may be required.
Most chronic medical conditions and prescription medications do not affect your eligibility. Bring documentation of your conditions and medications. Dr. Manghnani will note them on the form. Certain conditions classified as 'Class A' (such as untreated active TB, untreated infectious syphilis, or specific mental health conditions with associated harmful behavior) can affect admissibility — we'll review your case and explain.
The I-693 immigration medical exam is generally not covered by health insurance because it is a non-medical, administrative requirement rather than treatment. Payment is collected at the time of service.
We accept Cash, Card (credit/debit), Zelle, and HSA (Health Savings Account). Most HSA plans cover the I-693 exam since it is a required medical examination.

We strongly prefer scheduled appointments so we can give each patient our full attention. We typically have same-week availability and often same-day for urgent cases. Please call before walking in.
If a follow-up visit is genuinely required (such as a second dose of a multi-dose vaccine series), there may be a small return-visit fee. We will always be upfront about whether a return visit is needed and what it will cost.
Yes. Dr. Manghnani is board-certified in Pediatrics, which makes her especially well suited for families with children. Children of any age applying for adjustment of status are welcome.
If your Form I-693 expires before USCIS makes a decision on your case, USCIS may issue a Request for Evidence (RFE) asking for an updated I-693. Schedule a new appointment with a Civil Surgeon as soon as possible — we can usually get you in within the week.
