What to Bring to Your I-693 Appointment

Bring as much of this as you can. Anything missing can usually still be handled in-office — but bringing it saves time and money.

We've pre-filled the official USCIS I-693 form with our practice information. Print it, fill in the sections you can (Part 1: applicant info), and bring it to your appointment. We'll complete the medical sections during your visit.

Required

  • Government-issued photo ID (driver license, state ID, or passport)
  • Your passport
  • Form I-693 (download the pre-filled form below and fill in Part 1 — we will complete the medical sections)
  • Payment for the exam fee

Strongly recommended

  • All vaccination records you have — from any country, in any language
  • A list of any prescription medications you currently take, with dosages
  • Any prior medical records related to chronic conditions
  • If you wear glasses or contact lenses, bring them

Helpful to bring

  • Results of any recent TB tests, chest X-rays, or blood work
  • Records of past surgeries or hospitalizations
  • Documentation of any mental health treatment, if applicable
  • USCIS appointment notice or receipt, if you have one

Don't have your vaccination records?

Don't worry — this is common, especially for applicants who grew up abroad. We have two options: (1) we can draw titer blood tests to check whether you already have immunity to certain diseases, or (2) we can administer the required vaccinations on the spot. We'll explain the cost of each option and help you decide.
